Saving a form error message: Either the username or the password is empty

I updated a form and when I hit SAVE I get the error message. Any idea how i can save my changes to my form?

Hi Kelli,

looks like this is a public form, right?
Go to the tab permissions and enter the credentials again for the user used for the public access.
