DocuWare on a domain controller.
In addition to Phil's response, on heavily used networks there can be the issue of port exhaustion: since most people also run DNS services on the domain controller, busy systems can actually use up all 65,535 ports. I've seen this several times, and unfortunately nothing can be done except for offloading the DNS traffic.
EDIT: Or, of course, give DocuWare its own server as suggested!

Unfortunately, importing DocuWare Request cabinets only works between equal versions of DocuWare. They are still usable as standalone archives, however.

If I were a betting man, I'd wager that there were Workflow Engine items stuck in an IIS cache somewhere, waiting to timeout: this module relies heavily on Platform and Settings app pools to do all of the configuration, designing, and operation of the workflows, and since there were some workflows in here before you even had the service itself installed, it may have encountered some undefined behavior.

Make sure that the correct SMTP connection is selected in the Workflow Engine Server section of the Admin Tool. Also, depending on your version there may be a hotfix for Workflow Engine notification issues. Note that even if you had installed the hotfix already, you may have to reapply them because the Workflow Engine would not have been installed when you did so.
You may also need to clear the Message Queues and restart IIS, as the task tab appearing there or not is controlled by messages in the bus. That being said, the task tab may not appear at first if you don't have a "Task" activity in your workflow: make sure you have one created, and that a workflow instance is waiting on a task step before we say there is definitely a problem.

"A newer version of DocuWare was found to be already installed"
David,
This error occurs if you've already started running DocuWare Setup for a newer version than you are currently trying to install: for example, you ran 6.12, realized you ran the incorrect setup, and then proceeded to try to install 6.7.
I suggest you open a support request at http://support.docuware.com so that we can get a technician to assist you with rolling back just the setup package.

We don't have anything specific to 6.9, but see https://www.docuware.com/support_faq/index.php?solution_id=4549 for more information on a known issue in 6.10 which may be related.
Also, check your DWSetupTrimmed log: if one of the entries is, "An error during installation. The module will now be uninstalled so that it is easier to fix.", then try https://www.docuware.com/support_faq/index.php?solution_id=5305
Finally, if none of that work, please open a support request so that we can take a look.
