Behavior:
Outlook starts in safe mode and the Connect-to-Outlook Add-In is not shown anymore.
Solution:
After running Windows Update the patch "KB3114409" breaks Outlook 2010 and stops Connect to Outlook from working.
The patch, which is designed to stop Outlook from starting in Safe Mode, actually does the opposite and forces a Safe Mode start.
To rectify, remove the Windows update patch. You may need to uninstall and re-install Connect to Outlook.
