that should be the functionality of the "Append" permission. Maybe the user has another profile assigned that gives him the right anyway?

at first glance your trigger looks fine, but I would suggest that you add the conditions one by one, so you can verify which of them works in your case.
So maybe just start with the first row and make sure, that the workflow triggers. Then add the next row and try again. Maybe there is really no document that matches your condition or maybe your workflows do not trigger, which means, that you probably have to contact support.

You have to use substitution rules in order to automatically reassign to the next user if the first one is out of office.
In the case specifically asked here there it is not about this automatic reassignment, so the solution suggested by Simon will work.

Hello Michaël,
I do not understand why the first person should lock out the others. Each user gets a single workflow task and can open the document. The only locking would happen, if the users have to edit the document or make annotations to it. Taking a decision with the workflow should work fine.

Actually the command just replaces all existing keywords with the new ones. So you have to read the current content and then write the current content plus the new content.

this post is old, but could you figure it out? If not I would be interested in understand what is slow, and why you think, that you cannot limit it far enough. Do you just need the data from a single column? Or just the data like it is shown in a select list?

auch wenn es möglich ist, dass ein Briefkorb von mehreren Benutzer gleichzeitig benutzt wird, gibt es für Briefkörbe keine Benachrichtigungen oder etwas ähnliches. Briefkörbe dienen der Vorsortierung und zur temporären Aufbewahrung zwischen dem Import und der Ablage in einem Archiv. Wenn Sie eine Benachrichtigung benötigen an die Benutzer benötigen, dann sollten die Dokumente in einem Archiv abgelegt werden. Dafür können dann auch die entsprechenden Benachrichtigungen erstellt werden.

"Allow new entries" is the right to add a completly new entry for this field into the file cabinet while "Modifiy" basically means, that the user can change the index entry for an already stored document. 
I hope this explains it better.
